Parent’s Agreement: I understand that I as a legal guardian of my child hereby expressly acknowledge that the activities involve accidental risks, and I do hereby voluntarily assume any and all risks such as injuries, disability, and death to my child and or my child’s property, which may occur from participation from these events. My child’s personal insurance bears primary responsibility in case of any accident. I release ELITE BASKETBALL ACADEMY INC, their volunteers, agents, officers, directors, partners, and coaches of all liabilities arising from this program. I authorize the business named above to charge the credit card on file indicated in this authorization form according to the terms outlined above will be charged monthly until cancelation is requested. If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day. I understand that this authorization will remain in effect until I cancel my membereship by filling out the cancelation form found on the website. I agree to notify the business of any changes in my account information or termination of this authorization with a minimum of 7 days prior to the next billing date. If cancelation request is filed after a charge you will be subject to a $15, Administrative fee. This payment authorization is for the type of bill indicated above. I certify that I am an authorized user of this credit card and that I will not dispute the scheduled payments with my credit card company provided the transactions correspond to the terms indicated in this authorization form.
