Elite Basketball Academy Inc. – Father & Child 3-on-3 Tournament Waiver and Payment Authorization

Parent/Guardian Agreement

I, the undersigned, as the legal parent or guardian and participant, acknowledge that both my child and or I a (father) will be participating in the Elite Basketball Academy Inc. Father & Child 3-on-3 Basketball Tournament.

I understand that participation in this event involves inherent risks, including but not limited to physical injury, disability, or death. I voluntarily assume full responsibility for all risks associated with participation for both myself and the rest of the team.

I acknowledge that in the event of an injury or accident, our personal health insurance policies will provide the primary coverage.

I hereby release and hold harmless Elite Basketball Academy Inc., its officers, directors, staff, coaches, volunteers, and agents from any and all liability, claims, or demands arising from participation in this tournament by myself or my child.

Payment Authorization
I authorize Elite Basketball Academy Inc. to charge the credit card provided in this authorization form for the **full registration fee** for participation in the Father & Child 3-on-3 Tournament.

I understand this is a **one-time payment** and that no recurring charges will be applied.

If the scheduled payment date falls on a weekend or holiday, I understand the transaction may be processed on the next business day.

I certify that I am an authorized user of the credit card provided and will not dispute the charge, provided it aligns with the terms outlined in this agreement.
